We are looking forward to seeing you for your first appointment with Felix R. Canout Rehabilitation Services, Inc. at one of our two locations: Beverly Hills P.T. and Sports Rehabilitation Center or Wilshire Center P.T. and Sports Rehabilitation. We encourage you to review this information as it will assist you with the office protocol and may answer questions prior to your appointments.

A new patient visit or consultation will require approximately 30-60 minutes. New patients are asked to arrive 15-30 minutes prior to the scheduled appointment for the proper completion of paperwork. This will allow you to be ready for your therapist at the scheduled time. Follow up visits be approximately 45-60 minutes, depending on the complexities of your problem.
If you are unable to keep your appointment, please call our office and reschedule at least 24 hours in advance to avoid failed appointment charges. By doing so, you allow those on our waiting list to be accommodated.
If you will be more than 5-10 minutes late for your appointment, please call to advise us. We respect your time and will make every effort to see you in a timely manner.
Beverly Hills: Parking is located behind our building. We have five parking spaces on the left hand side. There is also metered parking near our facility (Burton Way and Robertson).
Wilshire Center: There is a parking structure located at the first floor. We do not validate parking. Metered parking on the side streets allow for up to 2 hours .There is $3.00 all-day parking available behind our building at the lot located on Ingraham and Witmer Streets.
METHOD OF PAYMENT
Payment of co-pays and deductibles is expected when services are rendered. When you have insurance, please notify our Front Desk Coordinator first so that the proper forms may be completed and billing procedures explained.
Due to the large number of different plans, it is impossible for our staff to know your individual insurance plans benefits. It is your responsibility to familiarize yourself with your specific insurance plan and notify our staff of any changes, but please feel free to ask any questions about insurance benefits with our billing office at 213-481-1515.
